These are administrative forms required for various events and actions.

Troop Activity Permission Slip   Word Document   PDF Document

Troop 497 Parental Consent and Medical Release must be completed for all Scouts before they can participate in the outdoor program. It should be submitted to Mr. Reidler.

Family Account Withdrawal Word Document   PDF Document

This form must be filled out and given to the Troop treasurer to request funds be withdrawn from the family account for valid expenditures.

Reimbursement Check Request   Word Document   PDF Document

This form must be filled out and given to the Troop treasurer along with receipts to be reimbursed for legitimate, approved, expenditures. This form is available at Troop meetings.

Trailer Mileage Reimbursement   Excel File   PDF Document

Complete this form to be reimbursed for pulling a trailer on Troop 497 trips.

Driver List   Excel File   PDF Document

This form can be used to track the drivers for trips. It contains a place for the names of the drivers and the amount of passengers going and returning with each driver.
